Frequently Asked Questions

Are your services confidential?

YES!  Confidentiality is extremely important.  We are HIPAA compliant and will not release any information about you without your written permission.  We will not disclose whether you are a client or not without your permission.

How much do your services cost?

Equality Care (Medicaid)

We accept Equality Care as payment in full for services.  We do not charge a co-pay for services.

Kid Care

We accept Kid Care as payment in full for services.  We do not charge a co-pay for services.

Insurance

We will bill your insurance company for all services except evaluations.  Insurance companies typically pay 50 - 80% of charges.  Check with your group plan for specific details of what your insurance company covers.

We are contracted as in-network-providers with the following companies:  Health Info Network, First Choice of the Midwest, National Care Network, CIGNA Behavioral Health, and WISE Provider Network.  Check your group plan to see if your insurance company is contracted with any of these networks.  If your insurance company is contracted with a network that is not listed above please call to see if we can become in-network providers with them.  We want to provide the most cost efficient services to our clients.

We are required to bill your insurance company our full fee.  We offer a discount based on a sliding fee to our clients directly to assist with out-of-pocket expenses but are unable to extend this same discount to your insurance company.

Sliding Fee Scale

We provide services on a sliding fee scale.  Your fee is determined by your income minus $1,000 for each person living in your home.  Our fee ranges from $10.00 to $90.00 depending on your income level.  We are willing to negotiate a fee that is agreeable to you.  We DO NOT want fees for service to prevent anyone from getting the help they need.  Please call even if you think you can't afford it.  We'll work with you.

We are unable to extend your sliding fee to your insurance company.  If you have insurance we are obligated to bill them our full fee, currently $90.00 an hour.  We will adjust your bill to your sliding fee once your insurance responds.

Service Contracts

We accept contracts from a number of agencies to provide services to clients including, but not limited to, Department of Family Services, Department of Vocational Rehabilitation, Workers Compensation, Veterans Administration, Educational Resource Center, and Children's Resource Center.  We also participate in Employee Assistance Programs (EAP).
